Form Template for Logging PPE Issued to Staff 

This is a simple Form Template for logging all PPE issued to individual employees, with listings included for:

  • Quantity
  • Date Issued
  • Sign Received

Alternative to F-HS1. Instead of having one form to log every employee and their PPE issued, you may prefer to have separate logs for each employee – this form is designed for this purpose.

This template is a Word Document and is fully editable. It can be implemented as part of a health and safety management system.

This document is an easy way of employers keeping an itemized record of what employees have been issued and the employees signing for the equipment to knowledge that the equipment has been issued to them, these can be kept across multiple sites and and then brought back to one location where trends in use can be assessed.
